Program Agreement

The services you are purchasing, how they are delivered, and your responsibilities and rights.

Seller: Weight2Lose Management Inc., operating as Weight2Lose® Medical Clinics.

Version: PA-2026.10.12.1

Revision date: 12 October 2026.

Applicability: Applies to an order only when this version is identified in its acceptance record and durable agreement copy. Previously accepted documents remain unchanged.

1. Your agreement and order

This agreement is between Weight2Lose Management Inc. (“Weight2Lose,” “we” or “us”) and the customer identified in the order (“you”). It includes your accepted order or personalized proposal and these documents:

Before payment, your order or proposal identifies your services, visit allocation and durations, program length where relevant, intended appointment frequency, price, approved discount, applicable taxes and payment arrangement. You can review these details and the applicable documents before accepting. We record the accepted order, agreement version and payment authorization, and provide a durable copy that you can save or print.

A human Program Director helps eligible patients choose a suitable program and prepares a personalized proposal. Enrollment and payment are arranged after the patient reviews and accepts the proposal and applicable program documents. Individual MD and ND appointments are available to existing Weight2Lose patients, subject to clinical suitability, practitioner availability and authorization where care is received. International enrollment remains subject to the location review and conditions in the International Services & Currency Terms.

An inquiry, free consultation booking or conversation with an AI assistant does not enroll you or replace agreement acceptance. The Website Terms of Use also govern your use of the website.

2. What your program includes

Your personalized proposal identifies the services included. If it includes a Signature Program, your selected Registered Nutritionist or Registered Dietitian leads that program, primarily through private video or phone consultations. Every Signature Program session-count option includes the same core features:

  • A comprehensive intake and personalized dietary plan, including meal-timing or fasting strategies when appropriate.
  • The scheduled consultations included in your selected package.
  • Practical nutrition resources and private support for accountability and everyday decisions.
  • Plan refinement based on progress, feedback and available clinician-reviewed bloodwork.
  • An administrative support pathway between appointments and care coordination with your consent.

With your consent, your nutrition practitioner and any separately engaged MD or ND may coordinate existing labwork review and relevant chronic disease care within their respective scopes and the visits purchased.

Individual MD and ND appointments provide only the visit purchased. Administrative support is not emergency care or unlimited clinical consultation.

3. Clinical roles and limits

Your selected Nutritionist or Registered Dietitian leads the dietary and lifestyle work. An MD or ND provides additional clinical care only when separately selected or arranged. Buying one clinical visit does not include ongoing physician or naturopathic supervision, further appointments or tests.

Medical Doctors assess medical concerns, review medication and bloodwork, and make prescribing or investigation decisions when appropriate. Naturopathic Doctors assess relevant health concerns, fasting suitability and bloodwork, and provide recommendations within their scope. Registered Dietitians provide regulated nutrition care. Each regulated practitioner independently controls professional judgment, clinical records, treatment decisions and scope of practice.

Clinical services depend on availability, suitability and authorization where you receive care. No prescription, test, referral, diagnosis, result, weight loss, remission, medication reduction or insurance reimbursement is guaranteed.

  • Do not change medication, fasting, diet or activity contrary to your prescriber’s instructions. Longer fasting is not standard, is unsuitable for some people and requires medical review.
  • Share clinical information only through the approved secure care environment, not public forms, checkout, ordinary email or public AI tools.
  • Website information, checkout and AI assistance do not provide individualized medical advice.
  • This service does not replace primary, specialist or emergency care. For an emergency, call 911 or your local emergency service.

4. What fees exclude

Unless specifically included in your accepted order, fees exclude additional professional appointments, prescriptions, medications, supplements, laboratory tests, imaging, diagnostics, devices, pharmacy costs, third-party apps, emergency care and treatment outside the agreed scope.

A selected MD or ND add-on includes the purchased consultation, not external testing or medication costs. Consultations included in your Nutritionist program or RD package remain included when delivered by the assigned practitioner in the selected role.

5. Program length and scheduling

Program Start Date: The date of your first completed intake, subject to the customer-delay rule below. Schedule your intake within 30 days after payment. If you delay, the Program Start Date is day 31 after payment unless we agree otherwise in writing. If Weight2Lose cannot offer the intake within 30 days, you may choose a later start or receive a full refund.

Program length: Complete the included visits within the term stated in your accepted proposal. Program length and visit frequency with each practitioner are tailored to your needs and budget, subject to clinical suitability and practitioner availability. Six to twelve months is a planning guide, not a minimum commitment; shorter programs and smaller combinations of visits can be arranged. Changing the pace does not add visits or extend the term automatically. Extensions require written Program Director approval. Existing patients keep their previously accepted usage periods.

Individual MD and ND appointments: Each purchase covers one visit, including a clinical visit added to a Signature Program. These visits do not create or extend a program term. Payment does not reserve a time; the clinic confirms scheduling and any follow-up eligibility separately. The appointment rules in the Cancellation & Refund Policy apply.

Appointment frequency is set in your care plan, subject to practitioner judgment, availability, holidays and reasonable operational needs. Visits cannot be transferred or converted to cash, subject to refund and statutory rights. A missed appointment or cancellation with less than 24 hours’ notice may count as used, except where law requires otherwise or we grant an emergency exception. Reasonable provider or scheduling changes must not materially reduce your promised service.

6. Services, prices and payment

Your personalized proposal identifies the practitioners and services included, visit allocations and durations, program length, total fee, any approved discount, applicable taxes, and the amounts and dates of payments. You receive these details before accepting the proposal or authorizing payment. The general website budget guide is not your quoted fee or payment schedule. Services not expressly included are charged separately only as agreed. Your accepted proposal and applicable agreement govern your purchase.

All prices and payments are in Canadian dollars (CAD). Any approved discount and applicable taxes are shown in your personalized proposal before payment. A discount does not add visits or extend the agreed care term.

6A. Individual appointments and personalized proposals

Individual MD and ND appointments are for existing Weight2Lose patients. Each purchase covers one separately priced visit, subject to clinical suitability, practitioner availability and authorization where care is received. Medical and naturopathic appointments are purchased separately from Signature Programs.

Suitability, availability and any follow-up eligibility must be confirmed. If the appointment cannot be provided, the clinic cancels and refunds the purchase under the Cancellation & Refund Policy. Independent payment for an MD follow-up does not create a new physician-only program.

A human Program Director confirms the practitioner mix and the visits included in your personalized proposal. An MD or ND is included only when expressly selected by the Program Director. The accepted proposal identifies each service and charge. An AI assistant does not decide clinical routing.

6B. Tax, payment arrangements and receipts

Applicable tax is calculated by Stripe from your billing address before payment, after eligible discounts. Prices and payments remain in Canadian dollars (CAD). The retained service tax classifications are applied by the tax provider and are not a promise that services are tax-free in every country. Your billing address is separate from where you receive care; service availability remains subject to the location and suitability rules in this agreement and the International Services & Currency Terms, where applicable. Any instalment-versus-full-payment tax rounding difference is disclosed in the order before payment.

Your accepted proposal states whether payment is in full or under an available finite payment plan, including the amounts and dates of payments. An average monthly budget guide does not authorize monthly charges or establish a payment schedule. For a Signature Program with a finite payment plan, only the Signature Program recurs; appointment add-ons are charged once upfront. Individual appointments are one-time purchases.

For a monthly plan, the accepted payment authorization covers saving the payment method with Stripe and automatically collecting the stated remaining payments. The plan has no automatic renewal and creates no new billing periods after the agreed finite endpoint. Billing ending does not establish that an unpaid balance has been paid and does not end the agreed care period. Previously accepted payment schedules continue under their agreed terms. Applicable statutory payment rights remain available.

Pay only through the secure Stripe checkout or staff-assisted payment process confirmed by Weight2Lose. Stripe provides the payment receipt; eligible completed-service receipts identify services actually completed and the practitioner who provided them. A package prepayment does not mean every included appointment has occurred. Confirm practitioner coverage and per-visit limits with your insurer; reimbursement is not guaranteed. International currency and payment arrangements are governed by the applicable International Services & Currency Terms.

7. Cooling-off, cancellation and suitability

Your Rights under the Consumer Protection Act, 2002. You may cancel this agreement at any time during the period that ends ten (10) days after the later of the day you receive a written copy of the agreement and the day all the services are available. You do not need to give the supplier a reason for cancelling during this 10-day period. In addition, there are grounds that allow you to cancel this agreement. You may also have other rights, duties and remedies at law. For more information, you may contact the Ministry of Consumer and Business Services. To cancel this agreement, you must give notice of cancellation to the supplier, at the address set out in the agreement, by any means that allows you to prove the date on which you gave notice. If no address is set out in the agreement, use any address of the supplier that is on record with the Government of Ontario or the Government of Canada or is known by you. If you cancel this agreement, the supplier has fifteen (15) days to refund any payment you have made and return to you all goods delivered under a trade-in arrangement (or refund an amount equal to the trade-in allowance).

The Cancellation & Refund Policy forms part of this agreement. If intake determines that the program cannot appropriately be provided, Weight2Lose cancels enrollment and refunds all amounts paid to the original payment method. Nothing in this agreement reduces statutory cancellation or refund rights.

8. Your responsibilities

  • Keep your contact information accurate and protect your secure-care access credentials.
  • Attend appointments or give timely cancellation notice.
  • Use program materials personally; do not reproduce or redistribute them except as permitted by law.
  • Treat staff and practitioners respectfully and use emergency services for emergencies.
  • Use the confirmed secure payment process and report suspicious messages promptly.

9. Service changes and interruption

Weight2Lose may make reasonable provider, scheduling, technology, or administrative changes that do not materially reduce the promised service. If Weight2Lose cannot materially provide the program or a purchased appointment, it will offer a reasonable alternative, extension, or refund for the affected unprovided service, as appropriate and required by law.

10. Liability

Nothing in this agreement excludes or limits liability for fraud, gross negligence, wilful misconduct, breach of confidentiality caused by failure to use reasonable safeguards, independent professional negligence, or liability that cannot legally be excluded. Nothing removes your rights against a practitioner responsible for professional services.

For a claim arising solely from the Site, administration, or this commercial agreement—and not independent professional negligence—Weight2Lose excludes indirect or consequential damages to the extent permitted by law. Its aggregate direct contractual liability is limited to fees you paid under the program during the 12 months before the event giving rise to the claim, subject to all non-waivable rights.

11. Governing law and concerns

Ontario law governs, and the courts of Ontario have non-exclusive jurisdiction, subject to mandatory consumer and other non-waivable rights in your residence or service location. Contact Weight2Lose first so the team can try to resolve a concern, but nothing obstructs a statutory complaint, privacy complaint, chargeback right, or other non-waivable remedy.

12. Contact, notices and existing agreements

For cancellation, email info@weight2lose.ca, fax 416-636-3236, or use another method permitted by law. A notice is effective when sent where applicable law so provides; keep dated proof. For assistance, call 647-559-4496. Billing questions: billing@weight2lose.ca.

This version applies to new agreements accepted under it. Existing patients retain their previously accepted proposal, prices, services, payment schedule and agreement unless a change is separately agreed in writing.

This document forms part of an order only when identified in its acceptance record and durable agreement copy. Administrative-contact and promotional choices are separate from agreement acceptance. Statutory rights cannot be waived. Earlier versions are available to affected patients at info@weight2lose.ca.
